ATTENTION: Parents/Guardians
This
is a new process for physicals for sports !!!!!!!!!!!!!!!
Students
will need only one physical for the school year.
If there is an injury or
illness during a sports season, then the student will have to be cleared
by their private physician.
The
enclosed forms
must be completed by the parent/guardian correctly and completely.
This packet must be completed and returned to the school
nurse by
TUESDAY
AM, May 27, 2008.
If you choose to take
your child to your family physician, please return sections 1, 2, and 3
also by May 27. Keep section 4 for your physician to complete.

are:
Physical Exams are scheduled to begin at 8:30 AM till noon
